The demise of a 13-year-old woman after she took a sip of Costa Coffee sizzling chocolate was attributable to a "failure to follow the processes in place to discuss allergies", a coroner has dominated.
There was a "failure of communication" between the workers on the Costa in Barking, east London, and the woman’s mom, East London Coroners’ Court was informed on Friday.
