An inquiry has discovered "systemic failures" contributed to the deaths of two younger individuals who took their very own lives while in custody in a Young Offender’s Institution, in Polmont, Edinburgh.
21-year-old Katie Allan was discovered useless in her cell in June 2018. Four months later, 16-year-old William Lindsay additionally died in custody.
The inquiry has made 25 suggestions to attempt to stop the same tragedy.
